The Healthier Living (or Chronic Disease Self-Management Program) is a FREE evidence-based program developed by Stanford University and has been shown to improve health and well-being of adults with different chronic conditions.

Culminating two years of hard work, states and their community-based partners throughout the country far surpassed their goal of reaching 50,000 Chronic Disease Self-Management Program (CDSMP) completers (individuals who attend at least four of the workshop’s six sessions). Since March 31, 2010, more than 70,000 individuals have completed CDSMP! Over 8,600 workshops were hosted at nearly 5,500 unique implementation sites. About 75% of CDSMP participants are over age 60, nearly 47% live alone, and more than 60% have two or more chronic health conditions.

Meets 2 ½ hours per week for six weeks

Relaxation techniques
Nutritious eating
Improving communication with physicians, family, and friends
